Job Profile Palliative care Team - Nurse (Bsc/ GNM) (Female Only) (Only Experience Candidate) Job Title: Palliative care Nurse Reports to: Palliative Care Field Services Director, FST Nurse Status: Full Time Purpose of the Job: Coordination of patient care by palliative care team Provide critical nursing support and education to palliative care patients and their informal caregivers in their homes /OPC through the palliative homecare team Work as part of multi-disciplinary palliative care team in ensuring excellent patient care services and carrying out quality improvement initiatives Essential Duties and Responsibilities: Coordination of homecare patient visits according to priority (homecare nurses) Provide nursing care/education to patients at home/in OPC Enrollment of new patients, assessment of their nursing needs and prioritizing their care Ongoing assessment of nursing needs of old patients and changing priority as required Discharge of patients according to criteria laid out Assessment of informal caregivers ability and their education in care of their patients Liaise with and assist doctor in symptom management Maintenance of patient record pertaining to nursing issues Maintenance of monthly record of patients visits and priority Maintenance of medicine stock record Assist counselor in bereavement support of families Work as part of a multi-disciplinary team and assist doctor and counselor as required Attend fortnightly coordination meetings at headquarters Participate in continuing education training programs for palliative care staff during coordination meetings Participate in outstation trainings as decided by FST Nurse and Palliative care services Director Assist in in-house training sessions as decided by FST Nurse Perform other duties as requested Adhere to the practice of confidentiality regarding patients, families, staff, and the organization. Additional Duties and Responsibilities Participate in events held by CanSupport for - Bereaved families (Remembrance Day) - Cancer awareness - Fundraising (Walk for Life). Job Qualifications: Essential- B.Sc. or GNM (General Nurse Midwifery) Course Desirable- 5 years' experience as nurse in hospital or community setting Computer Skills Job Profile Palliative care Team - Doctor for Jamshedpur Purpose of the Job Provide critical medical support for pain and symptom control and other palliative care needs to palliative care patients through the palliative care team Work as part of multi-disciplinary palliative care team in ensuring excellent patient care services and carrying out quality improvement initiatives Essential Duties and Responsibilities: Daily visits to patients homes/patients in OPC to provide consultation in palliative care, symptom management and supportive care to the patient and to meet the general medical needs of the patient to the extent that these are not met by treating physician/oncologist Facilitate clarification of patient and family goals of care Consult with the team to establish a written plan of care Educate patients and/or caregivers on patients condition, progress of disease and end-of-life care and issues Maintenance of patient record Maintenance of Morphine record Work as part of a multi-disciplinary team and assist nurse and counselor as required Education of staff and coworkers Attend fortnightly coordination meetings at headquarters Participate in continuing education training programs for palliative care staff during coordination meetings Provide community education Participate in outstation trainings as decided by Palliative care Director Assist in in-house training sessions as decided by Palliative care Director Perform other duties as requested Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.
