Job Description
Qualification - BAMS/MBBS
Experience - 2 - 4 Years
- New admissions, including emergency admissions: Initial assessment comprising of detailed History, systemic and local examination, documentation of the same, informing the concerned speciality registrar and admitting consultant, documenting their instructions on patient file regarding allied investigations, references and plan of care.
- Perform the initial assessment of each patient upon admission within an hour of patient reaching ward.
- To take daily rounds independently during assigned shifts with proper documentation in progress sheet.
- Routine rounds and coordination with the consultants and documentation of their instructions/orders.
- Transcribe the daily medication orders in Drug chart as per NABH guidelines and ensuring that the consultant has validated it. (signatures of consultants in assigned space in Drug chart)
- To enter the details of all patients in the ward. Discharge module on a regular basis and update till day of discharge.
- To follow up with all the investigations to be done and trace the results.
- To coordinate with the consultants for discharge.
- Prepare discharge summary and get it verified and signed by RMO/Specialty Registrar/Consultant.
- To coordinate with diagnostic departments, correct requisition to be ordered and consents to be facilitated as per requirement.
- To coordinate with Emergency department in case of any medico-legal case.
- To maintain a record of shift-wise doctor to doctor handover communication.
- To keep update of all the clinical status/investigation reports of all the patients
- To communicate with the consultants in case of critical clinical conditions/investigations for immediate actions.
- Perform minor procedures at the ward whenever required (e.g. wound dressing, suture / catheter insertion/removal, etc.)
- Address and manage common medical illnesses with instructions from RMO/ Registrar/ consultants (e.g. acute injury, pain, common cold, etc.)
- Facilitate internal/external transfer of patients with necessary documentation
- To be available for Camps arranged by the hospital.
- To write Pre - authorisation forms, facilitate TPA approval in coordination with the TPA department.
- Flexibility in doing Rotational Shifts as per the Hospitals needs.
