OBJECTIVE : Deliver short-cycle, evidence-based counseling that keeps patients /started, adherent and confident** on their prescribed therapy while data is fed back to the broader care team to maximize clinical outcomes and minimize drop-out.

Location:Hybrid (85 % Site Based, 15% Home Visit ) 

Reports to: PSP Regional Manager.

QUALIFICATIONS:

• Bachelor’s in home science,  psychology, counselling, social work, nursing, or health education.
• 1 year counselling or case-management experience in healthcare.

CORE RESPONSIBILITIES

| 1 | Intake &  Screening –  Start  with the  explanation  of the patient on the   program , followed by the   consent. Once  the   patient  consents with his  signature on the   Consent Form  ,  explain the  program. Counsel  &  guide  the patient  on  therapy ,  product awareness ,  life style  modifications.   Conduct  awareness  programs  at the  doctor’s  clinic . Present  progress  of the patient  to the  HCP.   Enroll  the patient.  

| 2 | Questionnaire & Goal Setting – Discuss  &   mention the answers  of the  Questionnaire & based on it  define  the   customized goal  plan for the patient.

| 3 |Structured Touchpoints – Deliver 1 X 15 mins   call  per week. solve side-effects, fear of needles, cost  undersrtanding, 

| 4 | Adherence Analytics – log every call in PSP CRM; flag missed doses, late refills, adverse events;

| 5 | PharmacoVigilance – Notify  the   AE as per the prescribed  channel of communication to  concerned  authorities within  24 hours .

| 6 | Quality & Compliance – Maintain 100 % documentation; hit program KPIs: 85 % retained at 3 month  80 % adherent, NPS 65.

| 7 |Team Huddles– Present data  to the  Regional Coordinator on  regular basis, Attend the   designated  training sessions as   and when assigned.

SUCCESS METRICS (QUARTERLY)

- Patient Enrollment  > 80  patients

- Patient adherence 80 % at 90 days.

- Patient-reported confidence score 8/10.

- Documentation closed within 24 h 95 %.

Working  days - Mon - Saturday
Sunday is off
Timings -OPD  OF the  Dr.
Total 10 Drs.  
Each day  you  will visit  2 drs   , one in morning & one in evening . Location is  dependant   based on the  Drs  clinic.
Put the  data  in the    app  provided  by us  and  it will be  evaluated  by your  reporting  manager.  
Your  reporting manager is a Regional Manager  who  reports  into a  National Manager.  
Renumerations  is  included  of  Salary  +  Travel  +  Incentive.
Incentive   you  get  once in  3 month  
Travel allowance  is Rs 150 per  day.
To Travel   to Mumbai  on  16th and 17th march. Tickets  will be  booked  by you  . You are  eligible for  3rd AC which  will be   reimbursed.
